COVID-19 Client Health Questionnaire and Liability Waiver
Please answer these questions truthfully and accurately.

-I have not been diagnosed with COVID-19 within the last 14 days.
-I am currently not awaiting results of a COVID-19 test.
-I am currently not experiencing symptoms related to COVID-19 and have not experienced these symptoms within         the last 14 days.  These symptoms are as follows:
                                        cough
                                shortness of breath
                               difficulty breathing
                                    sore throat
                                        nausea
                                       vomiting
                                       diarrhea
                                       tiredness
                                         chills
                                body/muscle aches
                                      headaches
                                      confusion
                           loss of taste and/or smell
-I have not been around or exposed to anyone who has COVID-19 or is exhibiting any symptoms of COVID-19             within the last 14 days.
-I have not travelled outside of the country or state within the last 14 days.
-If I begin to show COVID-19 symptoms within the next 14 days after my appointment, I will notify my stylist.
-I have read and will follow all Paisley Salon COVID-19 guidelines to keep myself, my stylist, and those around me     safe.  Refusal to follow these guidelines will result in my appointment being rescheduled.

 *By replying with the word CONFIRM to this text message, you are declaring that you are truthfully and accurately answering YES to all of these statements.  Texting CONFIRM also acts as your agreement to release Paisley Salon and all of its stylists from all liability in the event of exposure to and/or contraction of COVID-19 during your services.  This release of liability extends to secondary exposure post services.

*If you answered NO to any of these questions, please contact your stylist and reschedule your appointment for at least 14 days out.

 Paisley Salon COVID-19 Guidelines

-All clients and stylists are required to wear an appropriate face mask for the full time that they are in the salon.                 *Behind the ear masks are the best for hair services.
-All clients must wait outside or in their car until their stylist texts or notifies them to come inside the salon.
-All clients will have their temperature checked and logged by their stylist prior to their appointment.  If the client         is found to have a fever of 100 degrees or more, they will be asked to reschedule their appointment.
-All clients and their stylist are required to wash/sanitize their hands before and after their appointment. 
-Everyone must practice COVID-19 social distancing while in the salon.
-We are asking that all clients come alone to their appointment, unless the client is a minor under the age of 12 and         requires a parent or guardian to be present or the client is elderly or disabled and requires a caretaker to be                 present.  In these special circumstances, we ask that only 1 parent/guardian/caretaker be present and they must           follow the same Paisley Salon COVID-19 guidelines.
-At this time, Paisley Salon is ceasing all customer beverages and magazine subscriptions and our waiting area will be       closed.
-Please limit the amount of personal belongings that you bring into the salon. 
-We are asking for clients to try and refrain from bringing in food or beverage, but if you need to, please make sure           to dispose of it on your own.
-During this time, only stylists will be allowed to pull product from the shelves, so please notify your stylist if you           need any help with products or would like to purchase any.
-When purchasing products, we are not allowing cash or checks at this time.  You may use a credit card or Paisley           Salon’s Venmo. @Paisley-Salon

         *Please ask your stylist what methods of payment they are allowing at this time.
